The Plymouth High School Athletic Department will be unable to schedule mass summer sport physicals for 2013. Please allow time to schedule your appointment. Forms can be picked up in the main office between 8am – 4pm Monday-Friday. Please enter Door #1. Please be advised individual sport physicals are due on or before August 2, 2013.
No student athlete in grades 9-12 will be permitted to practice without the following forms on file in the Plymouth High School Athletic office:
1. Physical
2. Emergency Medical Release
3. Code of Conduct
4. Drug Testing Policy
5. Concussion
