Pilgrim ManorEvery nursing home in the state of Indiana has an annual survey and it is a rarity to get a deficiency free survey.

The Indiana State Department of Health (ISDH) looks at the quality of nursing care, social services, activities, staffing, qualifications of staff, and the environment when conducting the survey.  They interview a large amount of residents, families and staff to see if they are happy and have choices and that their rights are in place.

The survey process is not only looking at how the facility is doing the week of survey, but looking at all the information from the past year.  In order to get a deficiency free survey, the staff has to be on top of everything at all times.

Facilities are rated on a 5 star basis, similar to the way hotels have a 5 star rating.   With a deficiency free survey, Pilgrim Manor is a 5 star facility in regards to the quality of care as the survey reflects.
